Combination Loop Archwire (CLA)
Broussard combination loop archwire는 .017 × .025 or .018 × .025 edgewise wire로 만들어
지는 specialized design된 arch form으로 space closing이나 bite opening or closing, curve
of spee leveling 등의 여러 가지 목적을 위해 사용된다.
(1) Formation of the CLA
1. 사용될 rectangular wire archwire의 gauge를 선택한다. (길이는 20-25cm) archwire상에 midline을 mark.CLA는 .016 × .022, .018 × .025 blue elgiloy wire로 만든다.
2. Edgewise turret 등을 사용하여 CLA를 ideal shape로 만든 후 구강 내에 적합 시켜 one lateral bracket의 distal edge에서 other lateral bracket의distal edge까지 사이의 길이를 측정.
3. CLA가 치아에 ligate될 때 access를 위하여 이러한 measurement에 2mm의 extraspace를 더한다.이 숫자 (측정치 plus 2mm)는 inter-lateral bracket space이다. 예를 들면38mm plus 2mm는 40이다.
4. Archwire에 직각으로 first bend를 한다. 그리고 나서 flat surface에 놓고 확인한다.
5. 반대편 archwire에서도 90도 bending을 시행한다. 이들 2개의 직각 bends는 midline의 왼편에 1/2, 오른편에 1/2에서 2개의 lateral bracket사이의 full curved space에 2mm의 extraallowamce를 plus한 것과 같다.
6. Archwire의 각 side에서 Tweed loop forming plier로 vertical bend를 만든다. 상악에서는 10mm, 하악에서는 8mm 이어야 한다.
7. Tweed plier의 smallest cylinder를 사용하여 archwire으 middle section에 평행하게 horizontal bend를 만든다.
이때의 길이는 3mm가 되게 한다.
8. 다음단계에서 archwire의 end는 tweed plier의 같은 position주위로 bend된다.
Wire를 midline쪽으로 되돌려 bend하고 다시 한번 이 end를 archwire의 middle portion에 평행 하게 한다.
이 bend는 LCA의 horizontal portion을 이룬다. 이때의 길이는 4mm가 되게 한다.
9. 동일한 plier를 사용하여 이 archwire의end를 직각으로 하방으로 bend 한다.
10. 다시 archwire의 end를 직각으로 midline으로부터 떨어지게 bend한다.
11. Archwire의 다른 한쪽에도 동일한 방법으로 시행한다. flat surface에 놓고 확인한다.
12. CLA가 torque에 사용될 때, middle portion은 약0.5-1mm가량 lateral end 보다 짧게 만들어져야 한다. 그래서 incisor region에서 1mm step-up을 가져야 한다.
C II elastics는 combination loop에 걸리며 archwire는 molar area에서 tip back되거나 cinch back된다.
