2) 30도 - 45도 buccal root torque applied to the lower molar
lower molar에서 passive torque 이외의 어떤 것도 differential crown to root movement를 야기할 것이다. 하악 구치에서는 하악 구치의 root가 buccal로 이동하는 buccal root torque와 하악 구치의 crown이 lingual로 이동하는 lingual crown torque를 구분할 수 없으므로 utility arch의 distal leg에 45도 buccal root torque를 줄때 root가 buccal 쪽으로 이동하는 양은 crown이 lingual로 이동하는 양과 비례한다. 하악 제 1 대구치의center of resistance는 frontal plane에서 cementoenamel junction 약간 하방에 있다. (Fig. 7). root의 buccal movement와 crown의 stabilization에 의해 buccal root torque을 부여하는 유일한 방법은 arch의 expansion이다. lower molar에 대한 cortical bone anchorage를 증가시키고 정상적인 악궁폭경을 조절하거나 유지하기위해서 utility arch의 distal leg을 장착 전에 충분히 expansion시킨다. (양쪽에서 각각 1 Cm 정도씩). orthopedic alteration에서 언급한 바와 같이 headgear의 inner bow 또한 하악 arch length에 큰 영향을 준다.
또 전후방적인 면에서 하악 제1 대구치의 buccal root torque는 molar root를 extemal oblique ridge 하방에 위치시켜 제 1 대구치의 center of rotation을 apex로 이동시켜 molar tip back시에 root의 전방이동은 적고 crown의 후방이동은 많이 되게 한다. (그러므로 molar crown의 후방이동을 적극적으로 시행할 때는 적절한 buccal root torque 완성 후에 tip back을 주는 것이 효과적이라 할 수 있다.)
3) 30 - 45도의 distolingual rotation
이는 발치 case에서 견치나 전치의 retraction시에 발생하는 구치의 mesiolingual rotation에 대항하기위한 조작이며 headgear 사용시 상악 구치부와 적절한 교합을 형성하기 위함이며 또 posterior occlusion과 overjet의over-treatment 개념에 입각한 조작이다.
